RABJJ Academy Robbinsville Liability Waiver & Assumption of Risk Agreement

In consideration of being allowed to participate in any way at RABJJ Academy Robbinsville NJ – RICARDO ALMEIDA TRAINING CENTER LLC DBA Ricardo Almeida Brazilian Jiu Jitsu Academy (collectively, "RABJJ Academy," "us," "we," "our," or "Company") programs or related activities, I, the student or parent/guardian of the student, acknowledge, appreciate, and agree that:

1. Risk Acknowledgment: The risk of injury from activities in this program is significant, including the potential for permanent paralysis and death. While skills, equipment, and discipline may reduce this risk, serious injury remains a possibility.

2. Assumption of Risk: I knowingly and freely assume all such risks—both known and unknown—even if arising from the ordinary negligence of the Releasees, employees, agents, or others. I accept full responsibility for my participation or that of my child.

3. Participation Responsibility: I agree to comply with all stated and customary terms and conditions for participation. If I observe any unusual hazard, I will remove myself or my child from participation and immediately inform a staff member.

4. Release and Indemnification: In consideration of participating in RABJJ Academy activities, I, for myself, my heirs, representatives, executors, administrators, and assigns, hereby release, indemnify, and hold harmless RICARDO ALMEIDA TRAINING CENTER LLC DBA RABJJ Academy, its officers, directors, employees, volunteers, agents, and insurers ("Releasees") from any and all claims or demands of any nature—past, present, or future—arising from participation or presence at the Academy. This includes injury, illness, death, disability, or property damage whether caused by negligence, recklessness, or intentional acts.

5. Health Statement: I certify that I (or my child) am in good physical health and have no medical conditions that would preclude safe participation in RABJJ Academy programs.

6. COVID-19 Notice: I understand that COVID-19 is highly contagious and may be contracted through person-to-person interaction. By entering the premises or participating in any activity at RABJJ Academy, I voluntarily assume all risks related to exposure to COVID-19. I agree to follow all health guidelines and protocols as required by the academy.

7. Media Release: I give permission for RABJJ Academy and its affiliates to use photographs or video footage of me or my child for promotional, advertising, and publicity purposes, including use on the internet and social media.

8. Travel & Exposure Disclaimer: I agree that neither I nor my child will attend in-person classes within 14 days of returning from any area identified by the CDC as high-risk for COVID-19 or following exposure to any individual suspected or confirmed to have COVID-19.

9. Medical Treatment Authorization: In the event of a medical emergency, I authorize RABJJ Academy staff to administer first aid and/or seek emergency medical treatment for myself or my child. I understand that I am responsible for all associated medical costs.

By signing below, I acknowledge that I have carefully read and voluntarily signed this Assumption of Risk, Release and Waiver of Liability, and Indemnity Agreement. I understand that I am waiving certain legal rights, including the right to sue.

If signing on behalf of a minor, I represent and warrant that I have full legal authority to do so.
